Bone responds to load, not to movement in general

Bone adapts to forces that are large, quick and unusual. Gentle continuous activity such as swimming or cycling is good for your heart and does very little for your hip and spine density. That is why walking alone, although worth doing, is not a bone programme. The forces that matter come from lifting something heavy and from impact: a heel drop, a hop, a firm step down.

What LIFTMOR actually showed

The LIFTMOR trial trained postmenopausal women with low bone mass twice a week for eight months using heavy resistance and impact work, supervised, with correct technique. The training group improved bone mineral density at the spine and the femoral neck, along with height and functional performance, compared with a low-intensity control. The result is important because it contradicts the common advice to keep loads light after a low density result, and because adverse events were rare under supervision.

What the wider review says

A Cochrane review of exercise for preventing and treating osteoporosis in postmenopausal women found a small but statistically significant effect on bone density overall, with the most effective programmes combining different types of exercise rather than relying on one. Read together with LIFTMOR, the reasonable summary is that exercise is worth doing for bone, that intensity matters, and that the effect on density is modest even when the effect on strength and balance is large.

The practical programme

Twice a week, four to six compound movements, five or six repetitions in a set that ends two repetitions short of failure, with the load rising over months. Add impact only if it is appropriate for you: ten to twenty heel drops or small hops, on the same days. Balance work on the other days, because a bone that never meets the floor never breaks, and fall prevention has better evidence than density change.

If you have been told you have osteoporosis

Get personal advice before starting, especially about spinal flexion under load, which is the movement most often restricted after vertebral fractures. Being told your density is low is a reason to train under guidance, not a reason to avoid loading. The women in LIFTMOR had low bone mass, trained heavy under supervision, and did better than those told to take it easy.

What food adds, and what it does not

Enough protein, calcium and vitamin D support the process; none of them replace the mechanical signal. Protein intake in older adults is often below what is recommended, and the PROT-AGE group advises more than the standard adult figure for people over 65, with more still for those who are ill or training. Supplements are a conversation for your doctor, particularly vitamin D, where the right answer depends on where you live and how much sun you actually get.

Does walking build bone after menopause?

Walking helps your heart, mood and general health, but on its own it applies too little load to change hip and spine density much. Heavy resistance work and impact are what bone responds to.

Is heavy lifting safe with low bone density?
